KC-40V Upgrade, Need Help

Have an 80's KC-40V (Superstrat-HSS style) with FR 600-S tremolo.  Am changing out the electronics including volume and tone pots, 5-way switch, and input jack.

I have purchased two Bourne 500K pots, a Switch-craft output jack and an Oak-Grigsby 5-way (single wafer) switch and a .022uf cap.  Strangely, the switch has six (6) yes, six connection lugs but only five positions. It's part number EP-0479-000 - 5-Way Oak Grigsby 2 Pole Switch...but with six lugs on both sides, not four.

What I need from you smart folks out there is a diagram of how to properly wire this beast.  I'm not an electrician!

I'd appreciate any help you folks provide.

Thanks for replying.
I actually have this diagram.  Problem is, the new switch is completely different from the this old one so I am stuck for the moment.
Hopefully someone will post the lug positions of my new switch and then I can move on to soldering and sounds again.
